SUBFUNCTION DEFINITION: Involves all aspects of the medication use process, including procurement, prescribing, compounding, dispensing, storage of medications, administration and monitoring. Partners with physicians and other clinicians to provide treatments and advice to patients, records patient history relating to medication use and ensures the safe and secure storage of medications and related supplies. Advises physicians in determining the best possible treatments for specific diseases, stays current on pharmacy regulations, treatments and developments in research and design. Pharmacists are the experts on drug therapy and the primary health professionals who optimize use of medication for the benefit of the patients. Meets educational needs of patients, healthcare team, students and residents. Ensures adherence to regulatory and fiscal requirements.

Leads efforts to improve documentation of the plan of care, care delivered, and patient and family outcomes.

Compliance Sustain a working understanding of regulatory requirements and accreditation standards. Support leadership in efforts to meet and sustain the regulatory compliance efforts of the department and hospital. Be mindful and judicious in monitoring safety and departmental policies and procedures. Be mindful of CCHMCs processes and compliance with CMS standards and freely communicate safety and compliance concerns to leadership team members.
Serve as a Resource Use current appropriate references to ascertain correct and complete answers to medication related
Continuous Improvement Perform quality assurance functions for the organization; operate and strive for the highest quality work. Provides structured education base on the plan of care goals and the needs of the patient and family; assists with peer and student learning experiences through mentorship and training activities; performs patient education as relevant to each call and situation.
Bachelor's degree in a related field
2 - 5 years of work experience in a related job discipline
Is eligible or has obtained Certification as a Specialist in Poison Information (CSPI)
