This exciting new position embraces both the CA and RA roles into one. Supporting attending hospitalists, internal medicine residency program, and advanced practice providers within Hospital Medicine, by promoting quality, service excellence, patient- and family-centered care, and maximize throughput by serving as the primary administrative, clinical support coordinating all non-physician tasks directly related to patient care such as scheduling or coordination of tests for same day/next day, medical records requests in coordination with bed placement, maximizes time for education, and post-hospital follow-up care prior to discharge. CA/RAs are central to communication, logistics management, and bridging the gap between the inpatient and outpatient settings by helping connect the dots between care team members and transitions of care programs. CA/RAs are impactful in quality improvement throughout the Divisions clinical operations by identifying barriers to discharge and care progression. This administrative, clinical role is high impact in a fast-paced clinical setting and currently responsible for thirty-two DOIM teams and continues to grow. Service locations include Michigan Medicine Main Hospital, Michigan Medicine-Trinity affiliates, hospital medicine staffed. Provide virtual support to the Hospital Care at Home (HCAH) program and soon within Michigan Medicine Pavilion Hospital
Serve as an important and valued member of assigned patient treatment team.
Assist with onboarding new team members.
Cross trained within in each role to provide appropriate back-up coverage as needed for seamless coverage.
Communication Flow Coordination to ensure uninterrupted continuum of care:
Attend daily interdisciplinary rounds either in day or evening.
Assist division leadership to be aware and ensure ACGME work hours are met, by continuing to complete administrative tasks allowing residents to focus on education.
Schedule all PCP and sub-specialty post-hospital follow-up appointments prior to discharge with goal to have all appointments identified prior to patients discharge.
Place referrals and discharge orders as requested by medical team.
Facilitate communication between hospitalists, APP, residents, and referring physicians.
Review newly admitted patients appointment schedule to address conflicts with inpatient stay and adjusts appointments to accommodate follow-up as needed.Create and update internal notes with patient lists for team dissemination for seamless coverage.
Participate with virtual rounds and communicate via secure chat via MiChart, completing closed loop communication, as needed.
Coordination of services:
Coordinate with Michigan Medicine scheduling departments to make follow-up appointments with patients meeting recommended follow-up dates utilizing Michart Inbasket request, secure chats, and phone calls per departments procedures.
Obtain test results and records as needed from referring hospitals, outside physicians and/or ancillary services.
Communicate with the Transition of Care Pharmacy to ensure insurance authorization for timely medical coverage and fulfillment at time of discharge.
Coordinate with CVC partners when cardiac monitoring hardware is needed for patients prior to discharge.
Coordinate discharge needs with referring providers which may include follow-up studies, testing, specialist appointment scheduling, prescription needs or other referrals.
Create and provide work, school, or hospital transition notes.
Collaborate with outpatient infusion scheduling (MIST) for patient therapy plans and care coordination.
Obtain pathology reports/slides, microbiology culture results and sensitivities, imaging and all other medical records from outside hospitals/facilities.
Send H&P and DC summaries to providers for continuation of care and provider satisfaction.
Establish patients with PCP (internal or external) verifying the patients insurance is compatible with clinic guidelines.
Establish patients with new sub-specialty outpatient clinics, adding specialist as part of the care team and updating after visit summary (AVS) with the correct demographic information.
Participate in quality improvement initiatives.
Report barriers as they pertain to continuation of care and discharge length of stay (scheduling, system, or resource shortages).
Monitoring long length of stay issues, updating long length of stay list and partnering with care management, social work and hospital leadership for discharge preparation.
Participate with new initiatives utilizing experience and operational knowledge as requested by DOHM leadership team.
Proactively identifying system issues and improving upon those within scope of responsibility.
Identifying workflow barriers and creating new standards practices within scope of responsibilities.
Assist with creating, maintaining, and sustaining workflow documentation for department.
Participate in monthly service meetings as needed.
Other duties as assigned.
High School Diploma.
Progressively increasing experience in a clinical setting, either inpatient or outpatient for three years.
Strong computer skill including Microsoft Outlook and MiChart applications along with exceptional written and verbal / interpersonal skills.
Excellent attendance
Must have demonstrated knowledge of medical terminology.
Highly motivated, strong organizational skills, ability to successfully complete multiple tasks within established and changing deadlines are required.
Must be able to work independently with minimal supervision.
The ability to complete multiple high priority items in a timely and thorough fashion is crucial.
Associate degree in a health science field, or an equivalent combination of education and experience.
Independent capacity to complete a detailed list of tasks. Identifies work needs and determines task priorities without frequent oversight to the satisfaction of physicians and patients in a timely fashion.
This position is 40 hours per week, located at the main Adult Hospital with standard hours of Noon- 8:00 pm
Training will be days, 6-8 weeks depending on experience from 7:00 am- 3:30 pm
