Our program features a robust ambulatory care pharmacy practice that encompasses key elements of primary care and family medicine pharmacy practice. There are two primary care clinics for longitudinal continuity experiences, each served by two full-time pharmacists that independently manage drug therapy under extensive collaborative drug therapy management agreements and bill for services provided.
Pharmacists routinely manage anticoagulation (warfarin), diabetes, hepatitis C, hypertension, COPD, and smoking cessation. They also perform spirometry, see patients for HIV consults and polypharmacy, and spend time at our pulmonology clinic providing smoking cessation services. Residents will also have the opportunity to precept PGY1 pharmacy residents and teach family medicine medical residents throughout their year.

The PeaceHealth Southwest Medical Center Pharmacy team is comprised of approximately 80 pharmacist and technicians. The Pharmacy Department at PHSW supports a variety of acute and ambulatory care areas with clinical pharmacy programs and services offered throughout the organization.
