Ambulatory Pharmacist
Froedtert Health is an ever growing organization focused on providing academic and community medicine to Southeastern Wisconsin. The Ambulatory Pharmacy Department at Froedtert includes primary care, specialty clinics, and anticoagulation clinic pharmacists. These pharmacists are pivotal for patients and the organization to achieve certain goals, and clinical outcomes. Pharmacists provide top of license clinical pharmacist care through collaborative practice agreements (ordering medications, labs and referrals), independently see patients in clinic and virtually, and bill for services. As the organization continues to grow, clinical pharmacist positions across primary care and the specialty clinics are being developed.
PGY2 Ambulatory Care Residency or equivalent ambulatory experience preferred
Wisconsin Pharmacist License obtained within 90 days of start date
Solid Organ Transplant Pharmacist
Solid organ transplant pharmacists at Froedtert are directly involved in the pre and post care of thoracic and abdominal transplant patients in the ambulatory and acute care settings. This includes pre-transplant assessments when needed, medication management and monitoring, patient teaching, drug information, and coordinating transitions of care. Pharmacists have a role in precepting students and residents and contribute to the development and maintenance of transplant related medication guidelines and collaborative practice agreements. Froedtert & the Medical College of Wisconsin is home to an established and large pharmacy residency program. PGY1 positions are available in acute care setting (10 residents), ambulatory care setting (4 residents), PGY1 community-based (1 resident), PGY1/PGY2 Health-system Pharmacy and Leadership (4 residents), PGY1/PGY2 Medication Use Safety and Policy (4 residents), PGY1&2 Pharmacy Informatics (2 resident). Additionally, three PGY1 resident positions are available at Froedtert Menomonee Falls, one of Froedtert's affiliated community hospitals.
PGY2 positions are available in a variety of specialties including ambulatory care (3 residents), critical care (2 residents), emergency medicine (2 residents), infectious diseases (1 resident), oncology (4 residents), thrombosis and hemostasis management (1 resident).
