Department Name: H3330:Pharmacy ServicesJob Summary: The Pulmonology Pharmacist will work as part of a multidisciplinary team providing care for patients who have chronic pulmonary conditions. They will work collaboratively to provide chronic care management services for conditions such as asthma, chronic obstructive pulmonary disease, and intestinal lung disease with physicians and advance practice providers. In addition, the pharmacist will see patients independently to manage medications for pulmonary conditions. The pharmacist will be expected to have or be willing to obtain certification in the area of practice. The pharmacist will be expected to contribute to the goals of the enterprise and provide documentation of quality of service.
