ACPHS currently has several PGY2 programs (https://www.acphs.edu/pharmacy-residencies). Each of these programs is a one-year post-graduate training programs that offer the resident a variety of clinical experiences as part of a multi-disciplinary care team. Available programs exist in ambulatory care, cardiology and geriatrics. The goal of these programs is to advance the evidence-based knowledge, skills and abilities of the resident in providing and managing drug therapy for patients and designed to provide residents training experiences to become leaders in pharmacy practice. Each program is a 12-month, full time curriculum to provide the resident with clinical insights and practical experiences in pharmacotherapy. These programs will build upon the training from PharmD education and PGY1 pharmacy residency programs to teach the resident to serve as an expert on the optimal use of medications used in the care of patients with a variety of diseases and optimize the outcomes of patients through provision of evidence-based, patient-centered medication therapy. Each program is described in greater detail below.
1. The PGY2 Geriatric Pharmacotherapy Residency at ACPHS is a one-year post-graduate program designed to develop leadership skills and provide training and experience in various areas of geriatric practice, research, teaching, and service. The resident will be responsible for providing clinical pharmacy services in various clinical settings. This will allow the resident to develop relationships with patients and practitioners and to become adept at clinical evaluation and medication management in older adults. A customized plan will be developed for the resident based on previous experience and interest areas. The resident will be involved with teaching, research, and service activities at ACPHS. Residents also participate in teaching activities and professional events for pharmacy students, and are expected to facilitate case-based workshops and serve as tutors throughout the training year. At the completion of the program, the resident will be prepared to pursue credentialing as a board-certified geriatric pharmacotherapy specialist and obtain a position as an advanced practice geriatric pharmacist or clinical faculty at a school of pharmacy.
2. The PGY2 Ambulatory Care Residency at ACPHS is a one-year post-graduate program designed to develop leadership skills and provide training and experience in various areas of ambulatory care practice, research, teaching and service. This unique program provides exposure to primary care and public health pharmacy, as well as specialty areas that include rheumatology, pain management, and nephrology. Core learning experiences include outpatient primary care, rheumatology, nephrology and hypertension, public health pharmacy, pain management, academia/teaching, and research/scholarship. Electives are selected based on residents’ needs and interests and may include community pharmacy within a federally qualified health center, medication therapy management, practice management, pharmacogenomics, and/or subspecialty ambulatory care pharmacy. The ACPHS Teaching and Learning Certificate Program is offered as an optional experience for residents who have not previously completed a teaching certificate. There are opportunities to participate in public health outreach, transitions of care, and in local and national committees and foundations. Previous residents have gone on to jobs in academia, ambulatory care pharmacy, managed care and specialty pharmacy.
3. The PGY2 Residency Program in Cardiology Pharmacy is a 12-month, full time curriculum divided into monthly and longitudinal experiences. It is designed to provide the resident with clinical insights and practical experiences in cardiovascular pharmacotherapy care. The program will teach the resident to serve as an expert on the optimal use of medications used in the care of patients with cardiovascular diseases with a focus on ambulatory care. The resident will gain additional training and experience through exposure to a variety of cardiovascular disease states, direct patient care, practice-based research, and teaching. PGY2 pharmacy residency programs build on PharmD education and PGY1 pharmacy residency programs to contribute to the development of clinical pharmacists in specialized areas of practice. PGY2 residencies provide residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care or other advanced practice settings. Residents who successfully complete an accredited PGY2 pharmacy residency are prepared for advanced patient care, academic, or other specialized positions, along with board certification in cardiology pharmacy (BCCP).

Have strong interpersonal and communication skills and a desire to provide patient-centered care to older adults in various practice settings.
Possess a Pharm.D. degree from an ACPE accredited School/College of Pharmacy.
Be eligible for a license to pharmacy in New York State.
Be a U.S. citizen or carry a permanent resident VISA.
Have completed, or expect to complete, a PGY1 ASHP-accredited residency program.
The resident must also complete their licensure within two (2) months of beginning the residency
