This residency is designed for flexibility to meet the training needs of the resident and produce a well-rounded, independent ambulatory care practitioner. The program consists of a minimum of 2000 contact hours in a variety of practice settings. All residents have core rotations in primary care (including internal medicine, family medicine), ambulatory care specialties, and acute care. Primary care rotations will be longer in duration (two rotations of 5 weeks and 3 weeks) to allow for continuity of care and development of extended patient relationships. Residents also have orientation, practice management and drug information rotations. Residents select their ambulatory care specialty rotations from a broad range of options including cardiology, gastroenterology, rheumatology, and psych. Others can be arranged depending on resident interests and availability. Residents become privileged under multiple collaborative practice agreements to allow for practicing at the top of their license. Each resident selects an advisor from preceptors for their major project, and a coach for the optional teaching certificate program. The staffing requirement includes staffing evenings and weekends. Graduation from ACPE-accredited college of pharmacy with PharmD or equivalent combination of education and clinical experience, letter of intent (personal statement) describing reason for interest in the University of Utah program, college transcripts, written statement on the applicant's experience with social determinants of health (<300 words - prompt below), and curriculum vitae. Applicants must have a minimum GPA of 3.1 on a 4-point scale. Residents are expected to obtain full or temporary licensure in the state of Utah prior to starting the residency year. Please submit all required PhorCas materials as early as possible. Resident must be eligible to work in the US prior to the start of the residency program as well as be fully licensed as a pharmacist by the Department of Professional Licensing in the State of Utah by Sept 30th of the program year.
