We seek a Clinical Pharmacist to perform professional duties and responsibilities associated with comprehensive medication management.
You’ll enjoy the flexibility to work remotely * from anywhere within the U.S. as you take on some tough challenges. This remote position can be located anywhere in the U.S. but requires a pharmacist license in Arizona. Schedule will be Monday-Friday, 8:00-4:00 PT/10:00-6:00 CT.
Primary Responsibilities:
Assesses medication regimen for indication, effectiveness, safety and compliance
Provides patient-centered care that will improve clinical outcomes and reduce total cost of care
Communicates individualized care plans effectively with patients and healthcare providers
Supports patients in their initiation and maintenance of new therapy; providing education, optimizing utilization and establishing adherence
Engages patients at clinically relevant times to deliver impactful clinical pharmacy services
Maintains department performance levels for quality and productivity
Performs other projects, work, and activities as assigned to ensure continuous service to our patients and clients
This position may be required to obtain multi-state licensure as necessary for business need

Required Qualifications:
Bachelor’s Degree in Pharmacy or PharmD
Current unrestricted active pharmacist license in the state of AZ
Preferred Qualifications:
Multi-state pharmacist licensure
Residency and/or Board Certification
