Mosaic Medical is a network of clinics in Central Oregon that are Federally Qualified Health Centers and five start patient centered care homes. The clinical pharmacy team is integrated into these primary care clinics as a part of a patient centered primary care home model. We provide comprehensive medication management through direct patient care utilizing collaborative practice agreements including diabetes, hypertension, anticoagulation, hepatitis C, and ASCVD risk reduction management. We also provide drug information, administrative, and strategic support services.
