The Pharmacy Care Coordinator is a hybrid of two components. The first component is inventory management; responsible for the supply closet which includes maintaining inventory, running reports, and tracking provider usage and documentation. The second component is supporting the clinical pharmacist; responsible for completing phone calls, faxing, and supporting clinical pharmacy programs.
Support the Pharmacist and market by assisting with medication therapy management, patient safety, and quality initiatives. This position will be responsible for the implementation and tracking of pharmacist medication optimization recommendations, triaging incoming pharmacy tasks and referrals, and executing pharmacy reporting. The Pharmacy Care Coordinator will serve as a subject matter expert on Health Plan formularies and pharmacy benefits. This position will also be responsible for the market medication and supply closet which includes maintaining inventory, expired medications, and tracking provider usage and documentation.
Work Schedule: Expected to work normal business hours between 8:30am-5pm Monday - Friday, no weekends, or holidays.

Primary Responsibilities:
- Responsible for maintaining and restocking medications and supplies for providers in assigned territory
- Organize and maintains a consistent stock of inventory by ordering and receiving stock
- Ensures accuracy of the digital vs the physical inventory on hand
- Ensure provider compliance by tracking medication and supply usage and documentation
- Maintain medical refrigerator, document daily temperatures, and download temperature reports. Provide support to the market during annual flu campaign
- Provide support to the pharmacist for clinical pharmacy programs by completing phone calls, faxing patients, providers, pharmacies and/or health plans
- Call and follow up with patients as directed by the pharmacist and will relay findings to pharmacist
- Contact and follow-up with community providers and prescribers via fax and phone regarding pharmacist recommendations, track status, triage pharmacy fax inbox for responses
- Contact community pharmacies to verify co-pays and/or to investigate medication access issues on behalf of patients
- Educates patients on pharmacy-based rules related to medication resources or assistance program
- Provide support and guidance to patients and the market on medication adherence by actively communicating pill packaging, home delivery, and mail order solutions
- Executes and maintains reports and mail patient education to patients
- Triage incoming Landmark Pharmacy tasks and referrals and track status
- Perform other duties and projects as assigned

Required Qualifications:
- High School Diploma or GED
- 6+ months of experience with Microsoft Office Suite

Preferred Qualifications:
- Active/current National Pharmacy Technician Certification (PTCB, NCCT, or ExCPT)
- Active/Current Pharmacy Technician State License
- Experience with geriatric or Medicare Part D
- Minimum one years of current/recent pharmacy or PBM experience
- Inventory management experience
- Medical or pharmacy background
- Bi-lingual experience, Spanish
