The Pharmacy Department uses a multidisciplinary approach to patient care, emphasizing therapeutic efficacy, and safety. We deliver comprehensive, clinical and dispensing services in both inpatient and ambulatory care settings through a decentralized satellite pharmacy services model.
We are looking for a confident, independent individual to join our AdultOncology Clinical Pharmacy Specialist teamin the following services:
Lymphoma
Leukemia
In this role, you will serve as a fully coordinated member of the inpatient services and ambulatory clinics to provide advanced clinical pharmacy services.
You Will :
Provide drug therapy design and monitoring, patient medication counseling, and participation in Collaborative Drug Therapy Management (CDTM)
Service as part of a committee to develop treatment guidelines and pharmacy policies
Guide the preceptorship of PGY2 Oncology Pharmacy residents and pharmacy students
Educate hospital staff through Grand Rounds presentations and in-service training
You Have:
PharmD (or equivalent)
NYS license (or eligibility)
A completed ASHP-accredited PGY1 Pharmacy Practice residency
A completed ASHP-accredited PGY2 Oncology
Hours:
Regular hours of work fall between Monday and Friday, 37.5 hours per week
Flexibility may be required depending upon the needs of the department
