Providence is one of the largest providers of healthcare in the nation and operates over 50 hospitals and 1000 clinics across the western United States. Our region, Providence Northwest Washington, is comprised of a large local hospital (Providence Regional Medical Center – Everett (PRMCE)) and ten PMG outpatient clinics located throughout Snohomish County, Washington. The PRMCE Department of Ambulatory Pharmacy’s Medication & Anticoagulation Clinic provides multiple clinical services to our communities, and partners with PMG clinics to increase patient access to these services.
Services currently offered by the Department of Ambulatory Pharmacy include anticoagulation management (ACC), diabetes management (DM), hepatitis C treatment (HCV), transitions of care medication reconciliation for discharging inpatients (TOC), refill authorization (Refills), and on-site medication dispensing through Providence’s partnership with Credena Health Pharmacies. Credena Health Pharmacies also offer expanded patient services, including immunizations, adherence packaging, medication therapy management services, home or bedside medication delivery, and other services, depending on site location.
