The Primary Care Patient Aligned Care Team (PACT) Clinical Pharmacy Specialist (CPS) in the ambulatory care setting must be a licensed pharmacist, preferably with a Pharm.D. or equivalent experience. The incumbent is responsible for all aspects of pharmaceutical care of assigned veterans in primary care patient align care teams (PACT) and as CPS cross coverage as needed for unassigned veterans.
