PGY2 Geriatric Pharmacy Residency Program
South Texas Veterans Health Care System
We are actively recruiting 1 qualified resident for this program. The residency program is designed to provide a training opportunity for individuals desiring to develop clinical competency in the specialized area of geriatric pharmacotherapeutics. The program incorporates both inpatient and outpatient rotations devoted to the development of skills essential for the delivery of pharmaceutical care with a focus on the older adult and complex patient care management.
The resident spends 8 months in core learning experiences to include acute, subacute, ambulatory and home base primary care, hospice, palliative care and transitions of care. The geriatric pharmacy resident is responsible for participating in the design and implementation of appropriate therapeutic regimens, monitoring the outcome of therapy, assisting in the application of pharmacokinetic principles to individualize therapy as needed and in the provision of patient, disease and drug specific information to other members of the health care teams. Elective opportunities are numerous in both ambulatory and acute settings.
The resident will be responsible for an independent residency project. The primary nature of these protocols center on care of the older adult.

U.S Citizen Pharm.D. degree from ACPE-accredited institution Licensure in Texas required if completing learning experience off-site Prior to starting, residents must have completed an ASHP accredited PGY1 Pharmacy Residency Program or have an ASHP PGY1 Pharmacy Residency Exemption. |
