Our Institution Plainview Hospital is a 219-bed acute care community-based facility located on Long Island, New York. Plainview is situated on the boarders of Nassau and Suffolk counties, approximately 30 miles west from New York City. In addition to medical, surgical and critical care programs, Plainview Hospital offers unique opportunities for residents to practice in various areas of acute care within the Northwell Health System, by choosing a four-week elective rotation at our affiliate hospitals throughout Long Island and Manhattan. Our Program The postgraduate year one (PGY1) pharmacy residency at Plainview Hospital offers residents the opportunity to accelerate professional growth beyond entry-level proficiency. The 12-month American Society of Health-System Pharmacists (ASHP) accredited program enhances skills in managing medication-use systems and supports optimal medication therapy outcomes for patients with a broad range of illnesses. Residents will experience unit-based pharmacy practice during the course of each clinical rotation and will be involved in direct patient care. They will work collaboratively with a pharmacist, medical team and other healthcare providers, and will serve as a drug information resource. Rotations will be tailored to the resident’s field of interest and ASHP requirements. This program prepares residents to assume positions as direct patient care clinicians, serve as adjunct or full-time faculty for a college of pharmacy or pursue a PGY2 residency in a focused area of practice. Faculty Staff and Preceptors Dennis Karagannis, MS, RPh Residency Program Director, Director, Pharmacy & Clinical Services Mr. Karagannis is the Director of Pharmacy & Clinical Services at Plainview Hospital. With over 20+ years of Hospital Pharmacy experience, Mr. Karagannis brought his knowledge gained from working at New York University Medical Center, St. Vincent’s Hospital and Long Island Jewish Medical Center to Plainview Hospital in 2010. Mr. Karagannis focuses on maintaining proper patient care standards of care and safety, in addition to engaging employees, inspiring team members and developing additional clinical programs within the department of pharmacy. Jonathan Falsetta, PharmD, BCPS Assistant Director, Pharmacy Clinical Education Services Dr. Falsetta is the assistant director of clinical pharmacy and education services. He received his PharmD from Arnold & Marie Schwartz College of Pharmacy and Health Sciences at Long Island University. He completed an ASHP-accredited PGY1 pharmacy residency at Long Island Jewish Medical Center and became board certified in Pharmacotherapy in 2012. Jaclene Dabbour, PharmD, MBA, BCPS Program Coordinator, Pharmacy Supervisor, Critical Care Dr. Dabbour is a pharmacy supervisor specializing in Critical Care. She received her Bachelor of Science in Molecular and Cellular Biology from the University of Arizona. Dr. Dabbour earned her Doctor of Pharmacy degree and MBA from Harding University. She completed an ASHP-accredited PGY1 Pharmacy Practice Residency at Northwell Health Plainview Hospital and became board certified in Pharmacotherapy in 2016. Maryam Ahmed, PharmD, BCPS Clinical Pharmacy Coordinator, Emergency Medicine Dr. Ahmed completed her Doctor of Pharmacy degree from St. John’s University College of Pharmacy and Health Sciences. She completed her PGY1 pharmacy residency at Plainview Hospital. Upon completion of her residency, Dr. Ahmed continued her journey at Plainview Hospital. She is currently a clinical pharmacy coordinator, specializing in Emergency Medicine. Hadeia Farooque, PharmD Clinical Pharmacy Coordinator, Emergency Medicine Dr. Farooque completed her undergraduate education at Stony Brook University, where she received a Bachelor of Science in Psychology. She earned her Doctor of Pharmacy degree from Long Island University. Dr. Farooque completed her PGY-1 pharmacy residency at Plainview Hospital where she is currently a clinical pharmacy coordinator specializing in Emergency Medicine. Jonathan Anson, PharmD, BCPS Clinical Pharmacy Coordinator, Transition of Care Dr. Anson completed his undergraduate education at Binghamton University, where he received a Bachelor of Science in Biology. He then went on to earn his Doctor of Pharmacy degree from University at Buffalo. Dr. Anson received his board certification in Pharmacotherapy and he currently specializes in Transitions of Care here at Plainview Hospital. Jerry Jose, PharmD Clinical Pharmacy Coordinator, Transition of Care Dr. Jose completed his undergraduate education at New York Institute of Technology, where he received a Bachelor of Science in Health Sciences. He earned his Doctor of Pharmacy degree from Long Island University. Dr. Jose completed his PGY-1 pharmacy residency at Plainview Hospital where he is currently a clinical pharmacy coordinator specializing in Transitions of Care. Current Residents Haley Fahnholz
