RESIDENCY PROGRAM OVERVIEWSwedishAmerican A Division of UW Health is a 333-bed, full-service, non-profit hospital serving the greater Rockford region, northern Illinois and southern Wisconsin. SwedishAmerican is also a teaching hospital that hosts the University of Illinois College of Medicine's residency program. Clinical pharmacists are assigned to the emergency department, each floor of the hospital, specialty pharmacy, cancer center, retail pharmacy, and the pharmacist managed anticoagulation clinic. The PGY1 Community-Based Pharmacy residency program at SwedishAmerican’s URAC accredited Specialty Pharmacy builds upon the Doctor of Pharmacy (PharmD) education and outcomes to develop community-based pharmacist practitioners with diverse patient care, leadership, and education skills who are eligible to pursue advanced training opportunities including postgraduate year two (PGY2) residencies and professional certifications. Program Information• Number of Positions: 2 • Start Date: Last week of June • Staffing/Clinical Pharmacy Services: One weekday and every other weekend • Annual Stipend: $47,000 • Annual Leave

Minimum Requirements • Doctor of Pharmacy from an ACPE accredited College of Pharmacy • Eligible for Illinois licensure
