Clinical Pharmacist Practitioner

Upstream Pharmaceutical Care is seeking qualified clinical pharmacists who can become Clinical Pharmacist Practitioners to work in physician offices providing Chronic Care Management. Chronic Care Management (CCM) services are designed for patients with two or more chronic conditions, whose quality of health will benefit from coordinated patient centered care. The primary goal for CCM is to reduce the effects of acute and chronic illness exacerbations, that directly impact patient outcomes. This is achieved by involving the patient in their own care, building collaboration between healthcare providers, and improving communication across all service lines and care transitions. The Clinical Pharmacist Practitioners at UpStream focus on the patient’s medication experiences to help improve their outcomes.
The Clinical Pharmacist Practitioner (CPP) would be responsible for Chronic Care Management of patients and will provide direct patient care within the provider practice. The CPP will collaborate with the patient’s primary care provider to optimize evidence based pharmaceutical care. The CPP will evaluate the patient for drug related problems and will use an EMR to review patient’s conditions, laboratory test results, current medication list and pharmacy claims data. The pharmacist will communicate discrepancies and drug related problems found with the provider. The resolutions to the discrepancy or problem that were discussed with the provider will be documented within the EMR. The CPP will develop appropriate care plans and follow up for patients based on individualized patient needs. Care is delivered within the context of a long-term health care partnership with the patient that emphasizes health improvement, wellness, and disease prevention.
The Clinical Pharmacist Practitioner will deliver excellent customer service utilizing Relationship Based Care to advocate for safety, patient preferences, and optimal communication through partnering with patients/families and colleagues. Ideal candidates for this position should possess excellent verbal and written communication skills. The CPP must have the ability to work collaboratively as a member on an interdisciplinary team, establish and maintain effective working relationships with other healthcare professionals, practitioners, patients and families.

Required: PharmD Degree; Current active license to practice pharmacy in North Carolina Preferred: Pharmacy Practice Residency accredited by ASHP; Completion of an Ambulatory Care PGY-2 Residency; BCPS certification; At least five years of Pharmacy experience working in the ambulatory setting if a residency has not been completed. Knowledge Skills Abilities: Extensive knowledge of pharmacy and its related subdisciplines (therapeutics, pharmacology, physical pharmacy), formulary development, drug use review, quality assurance, legal, regulatory, and standards of pharmacy practice-good written and verbal communication, the ability to integrate healthcare team. Experience in medication reconciliation process. Interdisciplinary department support for education and monitoring patient drug therapy and education. Additional Requirements: Proficient at working with Electronic Health Records Systems. Working knowledge of the use of enterprise data warehouses and patient registries. Computer skills including Microsoft Word, Office, Excel, and PowerPoint. Works independently to meet expected deadlines for reports, projects and presentations. Must possess excellent organizational skills. Understanding of the principles of clinical integration and population health management. National Provider Identifier (NPI) is required. |
